At the Door Program provides supportive housing for HIV+ ex-offenders leaving the prison system. The program provides residents with a continuum of care from transitional housing through self-sufficiency. Supportive case management focuses on access to healthcare, substance abuse recovery, education, employment and services activities that can successfully connect them to their community. Candidates for At The Door have been homeless Baltimore City residents, with low- or no income and will no family or other support systems.
Application process normally begins during the pre-release period, through outreach events throughout the State of Maryland. One may also apply through their case manager, if they are in the first 3 months of transitional housing post release, and meet all other criteria. Clients may apply directly to permanent housing component once income and identification documentation and a personal recovery plan are in place. Required paperwork includes copies of birth certificate, social security card, proof of household income, and proof of qualifying disability. Applicants must be willing to accept case management services and be considered capable of living independently.
